Being so happy you could die might not be an exaggeration.
A 65-year-old mom was so overjoyed after watching her daughter get married that, just three days later, she was rushed to the hospital with chest pain and shortness of breath.
Doctors initially believed she was suffering a major heart attack. But what they ultimately was something “exceptionally rare.”
A new case study published in Oxford Medical Case Reports details the bizarre medical mystery.
When the woman sought care, doctors noticed she had been experiencing chest pain for several months, with symptoms worsening after the wedding.
Blood tests appeared to show a major heart attack caused by a blocked artery in the front-left portion of her heart, prompting doctors to “urgently” refer her to the emergency department, the case study authors wrote.
But there was one problem: no blockage could be found.
Further testing at the hospital showed her symptoms had seemingly resolved, and tests performed in the Cardiac Intensive Care Unit showed “the results were surprisingly normal.”
Perplexed, the doctors ordered an MRI, which finally revealed the culprit, a “ballooning” on the left ventricle, pointing to an “exceptionally rare and diagnostically challenging entity”: Happy heart syndrome (HHS).
A lesser-known form of Takotsubo syndrome (TTS), this condition occurs when an intense positive emotional experience — like a child’s wedding — temporarily weakens the heart muscle, preventing it from pumping normally. The characteristic ballooning spotted on the woman’s MRI happens when a sudden, severe surge of stress hormones, similar to adrenaline, stuns the heart muscle and causes it to change shape.
Because TTS closely mimics Acute Coronary Syndrome — reduced blood flow to the heart — it can easily be mistaken for a heart attack, making diagnosis particularly challenging.
The better-known version of the condition, often called broken heart syndrome (BHS), is triggered by intense negative emotional or physical stress, such as surgery, divorce or the death of a loved one. A 2025 study found that men were twice as likely to die from BHS, with 11.2 percent of men dying compared with 5.5 percent of women.
Happy heart syndrome, however, is particularly rarer, making up only about 4.1% of TTS cases. It may also be more challenging to recognize, because patients often present with milder symptoms, though researchers say more data is needed since the exact mechanisms behind TTS are not fully understood.
While the condition can be deadly, it is also reversible with prompt, appropriate treatment.
The woman was discharged home with “optimized medical therapy” and scheduled follow-up appointments. She resumed her usual daily activities and ultimately achieved “complete cardiac recovery.” According to the case study, she is now “looking forward to witnessing the happiest milestones of her other children in the years ahead.”
Despite growing awareness since TTS was first reported in 1983 at Hiroshima City Hospital in Japan, the condition remains under-diagnosed and frequently mistaken for a heart attack.
The case study reported that only 1% to 3% of patients who are initially suspected of having a heart attack but have no blocked artery are ultimately diagnosed with TTS.
“Maintaining a high index of suspicion, even in patients without conventional cardiovascular risk factors, is crucial,” the case study authors wrote.
“With timely recognition and supportive care, patients typically achieve complete recovery, highlighting the reversible and favorable nature of this unique condition.”
